Clinical Care Navigator Job Description
This position is a key team member supporting the Care Coordination efforts of our nephrology healthcare providers. The successful applicant will join our care coordination team as a full participant in preventive health care. Specific responsibilities include deploying the tools developed by CKP to capture key patient metrics of health care outcomes in your patients. The HCAS will be responsible not just for data collection but also for enabling the flow of data & analysis between our cutting-edge analytics system and the clinical care coordinators who use the analytics to improve patient outcomes.
We believe value-based-care is not only the future payment model of patient-centered medicine, but the way to enable physician groups to deliver the most effective care possible to improve patient outcomes. Care coordination at the grass-roots level is key to this effort – and the role of our Care Coordination Analytics Specialist is a key to care coordination.
Successful Applicants Will:
Be committed to improving patient outcomes
Be interested in a full-time position and looking to gain practical, paid experience in a patient care setting prior to applying to medical, dental, nursing, or physician assistant school
Solid understanding of information technology and the ability to learn the user interface of an unfamiliar IT system
Good communication skills
A willingness and drive to learn quickly
Ability to take direction and be a member of a team providing patient care
Excellent job attendance
Compliance with all medical and practice policies, including HIPAA
US work authorization
Radiating positive energy to our patients
Responsibilities:
Deploy Surveys & track each Patient attributed to one of our VBC programs
Understand & facilitate the data needs of our Care Coordinators and providers
Support Care Coordinators’ Responsibilities, including:
Patient Education
Modality/Access
Transplant Education
Appointments/Follow-up
Transport Help
“Hello Visits” within 6 months of the start of dialysis
Consultation with the care team and other providers on patient needs
Other duties as assigned
