Carolina Kidney Partners and Conduce Health partner in Innovative Value-Based Approach to Improve Outcomes for Medicare patients with Chronic Kidney Disease (CKD)

Carolina Kidney Partners (CKP), a multi-state, physician-owned, physician led organization and Conduce Health, a value based multispecialty care management company announce an innovative partnership. Chronic kidney disease impacts 23% of Medicare patients. Many of whom start unplanned dialysis, which results in a drastic impact to quality of life and 25% increased mortality. All eight physician groups with more than 140 providers united under CKP are fully committed to provide an enhanced level of care through a value-based approach that rewards preventative, evidence-based care. The goal of this new initiative is to improve the outcomes of patients with chronic kidney disease.

CKP will be responsible for care management of patients with chronic kidney disease in South Carolina, Georgia, and growing geographies. That includes responsibility for best in class data analytics and care coordinators embedded in each nephrology practice location. CKP will offer personalized patient care including patient outreach, patient education and transplant referral. All of which supports a multi-faceted approach that focuses on delaying progression of kidney disease, improving the transition to dialysis, increasing the use of transplantation, and increasing the number of patients with home dialysis.

Partnering with Conduce Health enables complex CKD patients with multiple comorbidities to be matched with the optimal nephrologist for them and enables these nephrologists to focus on delivering high quality care and participate in value-based arrangements.

The partnership with Nephrology & Hypertension Medical Associates expands the footprint of Carolina Kidney Partners in both the commercial insurance market and Medicare. Through value-based agreements, an approach that rewards agreed upon quality metrics rather than fee for service, CKP’s fully committed physician groups anticipate being able to provide an enhanced level of care leading to improved outcomes for patients with chronic kidney disease.

CKP will be responsible for care coordination at Nephrology & Hypertension Medical Associates as it is for all CKP practices. This includes responsibility for best in class data analytics, practice embedded care coordinators and value-based contracting. Care coordination will include patient-centric outreach, patient education focused on home dialysis modalities, transplant, and behavioral determinants of health. Hoping to achieve higher quality care at a lower cost by prevention rather than treatment, Carolina Kidney Partners (CKP), a five practice, physician-owned organization in South Carolina and BlueCross BlueShield of South Carolina announce an innovative partnership. Through a value-based arrangement, which is an approach that rewards agreed upon quality metrics rather than payment for individual tests and treatments, CKP’s fully committed physicians’ groups anticipate being able to provide an enhanced level of care and improved outcomes to commercial BlueCross members who have chronic kidney disease.

CKP will be responsible for care management of patients. That includes responsibility for best in class data analytics and care coordinators embedded in each practice location. Other material oversight of personalized patient care could include more intensive patient outreach, patient education and nutrition counseling. All of which supports a multi-faceted approach that focuses on delaying progression of kidney disease, improving the transition to dialysis, increasing the use of transplantation and increasing the number of patients with home dialysis.

According to the South Carolina Department of Health and Environmental Control, it is estimated that there are several thousand people diagnosed with chronic kidney disease in South Carolina, with the disease disproportionately affecting African Americans. The most common cause of kidney failure is diabetes, and the second leading cause of chronic kidney disease is uncontrolled high blood pressure.

Carolina Kidney Partners (CKP) announces the addition of Michael Marmor to the CKP management team as Chief Technology Officer.

Charleston, SC August 14, 2020

Michael Marmor has 26 years of software engineering and technical team management experience across a broad spectrum of sectors, including commercial software, telecom, internet, education, global nonprofit, medical, and financial services. Michael has been a principal in multiple fast-growing startups and a technical advisor and CTO for numerous early-stage technology companies. He holds a US patent for his work in peer-to-peer networking and a CLIO award recognizing technological innovation and creative excellence for a major global retail brand. Michael is a specialist in software development outsourcing and has worked with 80 nearshore and offshore software development firms in Asia, Latin America, and Eastern Europe on over 100 projects over the last two decades. Michael is a graduate of the University of Kansas but now lives in Charlotte, North Carolina, with his wife and young son. He is a dedicated yoga practitioner, an open-source contributor, and a fan of acoustic bluegrass music.

CKP is a physician owned value-based-care organization based in South Carolina.  CKP physician groups are spread across S.C. and allow access for almost the entire population of S.C. to nephrology care. Carolina Kidney Partners (CKP) announces provisional acceptance by the Centers for Medicare & Medicaid Services (CMS) for the Comprehensive Kidney Care Contracting (CKCC) option of CMS’s Kidney Care Choices (KCC) Model.

Charleston, SC June 15, 2020

CKP, a physician owned value-based-care organization in South Carolina, has been provisionally accepted by the Centers for Medicare and Medicaid Services for the CMMI/CKCC option. CKP has partnered with physician groups across S.C. in a first of its kind arrangement with CMS aimed at improving renal patient outcomes through a Value-Based-Care model. The goal is to engage CKP’s fully committed physicians’ groups to provide a new level of care to CMS patients in S.C. with chronic kidney disease (CKD). CKP will be responsible for the comprehensive care of a material portion of CMS’s CKD patient population in S.C..

“We are eager to get started; our agreement with CMS will free CKP’s physician partners to deliver a new quality of care to their patients,” said Dr. David Mahvi, CEO of Carolina Kidney Partners. “Under this contract, physicians are free of fee-for-service restrictions and can focus on delivering the best medicine to their patients.”

CKP will work with CMS to participate in performance year one, which begins April 1, 2021. CMS is making a second round of applications available during the spring of 2021 (1/1/2022 start) for additional Kidney Care Entities (KCE’s) interested in participating in the CMMI program.
